Policy Conversion Form, Please
Transfer Our Account to Group 919
We are following up
with all CDTOA members old and new to make sure they
are not in any other types of groups other then our Group
919. Betty Plowman, CDTOA Membership Service Director is making sure any new members
who may be in any other groups understand the transferring issue.
We will provide members with a form
letter which will allow you to transfer from other
groups to 919 within 90-days.
Members can fill out the PDF document form on the link below.
The PDF form has been set-up so all a member has to do
is add their company’s letter head information,
(logos can’t be inserted), type in the date on
top and company name, owner and address at the bottom.
Than print-out the letter, sign and mail it or fax to Armstrong & Assoc. (530) 668-2779 Fax
